Clinical Requirements
Students in the Health Sciences programs will be involved in clinical learning situations in which they will interact with consumers.  Governing licensure boards, Healthcare organizations and facilities have established appropriate requirements to maintain the safety of the consumers.  Prior to entry into the program, students must demonstrate that they have met the clinical requirements that have been established. 

Once a student has been advised that a position is available in the program to which they have applied, there is a process for completing all of the clinical requirements necessary for the programs. All of the clinical requirements must be met prior to finalizing the enrollment process. Instructions with the necessary forms have been put together to assist the student in completing and submitting the documents for meeting the clinical requirements.   These instructions and forms have been compiled into packets so that students will be able to access information and download necessary forms.

  • Health Form  - all Health Sciences programs (applicant questionaire and form for healthcare provider to meet physical examination requirement)
  • Immunizations (documentation form for immunizations)
  • Confidential Clinical Application all Health Sciences programs (for drug screening and criminal background checks)
  • Technical Standards - Nursing Programs (questionaire for student to assess their ability to meet physical requirements of nursing programs)
  • Technical Standards - Dental Hygiene (questionaire for student to assess their ability to meet physical requirements for the dental hygiene program)
  • Student Data Form - Associate Degree Nursing (data required by Board of Registered Nursing from ADN students)
